We accept:  check or money order.

2. TYPE in your information, then PRINT and SIGN the registration form.

Note: FILL IT OUT COMPLETELY as all information is needed to either report your completion to the state or process payment. (If a third party is paying for you, make sure you list their information in the payment section so the credit card number matches the name.)

3. FAX IT TO US at (561) 747-8683. Note: Do not e-mail the form as it has your personal information on it and may not be secure.

Refund Policy
There are no refunds, but we do allow you to reschedule certain courses.  Details are listed on the second page of the registration form; you may review it before you register.
